The term CareSmartz360 Solution “Solution” includes CareSmartz360 website, Cloud-Based Application, CareSmartz360 mobile application for smartphone and tablet (iOS and Android), any third party software provided with this license, any services offered and includes any accompanying documentation (the “Documentation”). Subject to the terms of this Agreement, Caresmartz Inc. (“Caresmartz”) grants to Subscriber, and Subscriber accepts, a personal, non-exclusive, and non-transferable license to use the Products and Services offered in accordance with the Documentation.
The Information We Collect

Including but not limited to the Demo link at the right corner of quick links tab – placed on top in the said website, wherein you can make requests to schedule a demo and the type of personally identifiable information collected through this link is: First Name, Last Name, Company name, type of agency, Email Address, Phone Number and a custom message box.
